The checklist questionnaire consists of items (or questions) that are answered with either 'Yes' or 'No'. The answers can impact the grade of a standard associated with a process.
Questions should be phrased positively, such that an answer of 'Yes' always indicates a pass, while an answer of 'No' always indicates a failure. For example: you want to validate a palette integrity. You should state your question as:
"The palette does not show external damages": A 'yes' answer assumes a positive grade
instead of:
"Do you see damages on the palette?": A 'yes' assumes a failure, however, according to the standard, this is deemed positive and does not adversely affect the grade. Consequently, it is crucial to frame responses negatively when they should affect the grade.
Checklists can be integrated in your inspections processes with the following dedicated step in the process flow:
standard/grade.
